Download Passport Application Form or D.E.P Form-1 and print 2 copies. Form two should be filled correctly.

What is required for e-passport:
1. Identity Card: National Identity Card or Online Birth Registration Certificate.

6. Passport Fee Deposit Receipt: ePassport Fee Deposit Receipt in Bank.
7. Address Proof: If the present address and permanent address are different then the citizen certificate of the permanent address should be provided. And present address commissioner's citizen card or certificate of employment institution or NID card (in case of employees), certificate of educational institution or student ID card (in case of students).
8. OLD PASSPORT: In case of re-issue of passport photocopy of previous passport should be given and original passport should be taken to regional office.
9. GD COPY: For reprinting of lost PASSPORT photocopy of passport and GD photocopy must be given and original GD copy must be produced.
10. Marriage Certificate: If applicable, Marriage Certificate or Nikahnama and Talaknama in case of divorce must be submitted. And NID CARD of husband or wife should be uploaded if unmarried in previous PASSPORT or voter ID card.
11. GO or NOC: Upload GO, NOC or Certificate, PRL Order or Pension Book of Govt employees, you will get urgent service and express delivery even after submitting the regular delivery passport fee.
Bank receipt, print copy of ONLINE application form, photocopy of NID Card or Birth Certificate, photocopy of NID card of father and mother in case of minor should be stapled together and submitted to Bangladesh Regional PASSPORT Office.
Original copy of NID CARD or Birth Certificate (may need to be shown) and photocopy while visiting Regional Office. In case of minors, original and photocopy of NID ID card of father and mother should be taken along with them.
